Dental Checkup Form Our school health program promotes overall health and well-being. As part of that program, we encourage parents to take their children in for regular dental examinations. If the form is returned by specified date the child will not be required to have the school dental inspection. |

Physical Form Students entering Kindergarten and 7th grade are required to have a physical before school begins. Please submit the physical, along with an updated copy of immunizations, to the school office. Parents are asked to keep school records up to date by submitting copies of any new immunizations. Records can be sent to the school office or faxed to 402-476- 8088. They can also be put in the weekend collection basket; please mark the envelope “School.” |
